Online Forms
Parental Consent Form We have to have an updated consent for every student we take on any type of trip. We will have a book of forms at the August 26 Parents' Meeting to update forms already on file. If your child does not have a form on file, feel free to download this form, fill it out, and bring in a copy to the church.
Confirmation Registration Form If your child is a rising 8th grader for the upcoming school year, he/she is eligible for the Confirmation class.
Reservation Form for Mission Trips
Italian Buffet
The Italian Buffet is coming up on April 18th, 2010 from 12:00 to 1:00. This Buffet Luncheon features Chicken Parmesan, Pasta with choice of Alfredo or Marinara, Salad and Garlic Bread with a dessert and beverage. All of the proceeds from this Luncheon will go to the ESUMC Youth Mission Trip to assist the Eleuthera Bahamas Methodist Habitat, an Outreach Ministry. The cost is $8 for adults and $5 for children.
