Parental Consent Form- We have to have an updated consent for every student we take on any type of trip. We will have a book of forms at the August 26 Parents' Meeting to update forms already on file. If your child does not have a form on file, feel free to download this form, fill it out, and bring in a copy to the church.
Confirmation Registration Form If your child is a rising 8th grader for the 2008-2009 school year, he/she is eligible for the Confirmation class. In order to get an accurate count of confirmads, please register your child no later than Friday, August 1, 2008.
